Over the past 21 years, and having consulted in excess of 7000 headache and migraine sufferers, I have developed an examination approach which not only confirms a neck disorder as the source of your headache or migraine, but if the techniques are used in a systematic way, the spinal segment (in the upper neck) can be identified. Temporary reproduction of headache and migraine pain when examining structures of the upper neck is recognised by The International Headache Society, The International Association for the Study of Pain and The International Cervicogenic Research Group as a key diagnostic feature of cervicogenic (neck) involvement in headache. However, my experience and neuro anatomical principles indicate that, reproduction of headache alone is not enough to confirm that the disorder is the cause of headache - there also needs to be lessening of the reproduced headache as the technique is sustained.
The examination techniques are applied slowly and smoothly (using the thumb) with the aim of temporarily reproducing your (familiar) head pain. It is important at this stage for you to know that if the thumb is taken off at any stage, the reproduced headache eases immediately, if not, within a few seconds. If reproduction occurs the technique is sustained for up to 90 seconds to achieve lessening of the pain. If this occurs then not only is cervicogenic (neck) involvement confirmed but also the specific spinal segment/s identified.
If on examination there are no relevant disorders to be found in your neck, treatment cannot be justified and would not be recommended. In this case we (practitioners*) would liase (with your permission) with your medical practitioner to discuss our findings.
If treatment is indicated it usually comprises passive movement techniques. These are applied in a slow and smooth manner and sustained ??? a 'persuasive' type of treatment aimed at restoring normal movement at the offending segment thus ensuring that the segment functions optimally ??? importantly you are in control at all stages during treatment.
The 'Watson Headache Approach' does not involve manipulation or 'cracking' of the neck. However practitioners* skilled in this approach are likely to be skilled in others and may choose to use high velocity thrust techniques or manipulation. Please discuss this with your practitioner.
We expect an appreciable improvement within five treatments and if this has not occurred then it is unlikely further treatment will be successful. The best possible chance to create a positive change in your headache or migraine symptoms is for those five treatments to be close together, that is, twice a week for two weeks and then again in the following week.

If, after any of the sessions, treatment is delayed, it is likely that momentum generated at the previous session will be lost. However if improvement is already evident within the first few treatments the scheduled treatments are reviewed. To justify ongoing treatment there needs to be recognisable, positive changes in your headache or migraine symptoms within this time.
